[Current]Telehealth: Remote Patient Monitoring — Discussion Paper
The purpose of this discussion paper is to provide a generic framework for computer-assisted and computer-based component activities supporting chronic disease management involving some aspects of remote patient monitoring, which are currently managed widely in Australia by hospitals, clinics and other care providers. This paper is intended to provide a fundamental ‘baseline’ foundation for further work on standardization and interoperability developments in the chronic disease management domain.
